These forms give us a better understanding of your medical and dental history so we can provide care that is personalized to you from the very beginning.
If you are transferring from another dental office, you can also complete a records release form so we can request your previous dental information on your behalf. This helps us get a clear picture of your oral health and ensures continuity of care.
